General Practice Management Plan (GPMP)

Patients with a chronic or complex condition such as heart problems, asthma, diabetes, mental health issues, lung disease, chronic back pain and others may be eligible for GPMP.

Our doctors and our practice nurse will help you to manage those health issues and you will be involved in a plan to manage your own health. This is directly billed to Medicare so there is no cost to you. The GPMP is reviewed after 6 months to evaluate any changes to your health or medication and to generally support you with your GPMP.

Team Care Arrangement (TCA)

TCA is available so that patients with those chronic conditions can access a wide range of allied health services, e.g physiotherapy, podiatry, etc at no cost or very reduced costs. This is directly billed to Medicare so there is no cost to you or a reduced cost.

The TCA is also reviewed after 6 months to evaluate any changes to your health or lifestyle and to generally support you with GPMP.
