Chronic Disease Management
Prevention is better than the cure – literally! Chronic diseases are complex and cause a great burden of disease to patients and their families. If managed well, they can often be controlled adequately and this can prevent complications and improve or prolong patients’ lives. Common chronic diseases include Diabetes, Osteoarthritis, Cardiovascular Disease, COPD (emphysema and / or chronic bronchitis) and Asthma. A considered chronic disease approach ensures that the patient’s regular doctor is taking a holistic approach to care provision and ensuring a team of other health experts is involved and consulted to improve outcomes and quality of life. This approach also allows eligible patients access to subsidised allied health visits through the Medicare system. Ask our team of doctors about how better to manage your chronic conditions.
