Family Doctor are looking for a passionate and motivated Full-Time Diabetes Educator to join our team at Rich River Health Group in Echuca, VIC.

About the Role
As a Diabetes Educator, you will play an integral role in supporting patients living with diabetes through education, management plans, and ongoing care. Working collaboratively with our General Practitioners, nurses, and allied health team, you will provide high-quality, patient-centred care to help improve health outcomes and empower patients to better manage their condition.

Key responsibilities include:

  • Providing diabetes education and support to patients and their families

  • Developing and implementing individualised management plans

  • Assisting patients with lifestyle modification strategies and self-management techniques

  • Working closely with GPs and the broader clinical team to coordinate patient care

  • Maintaining accurate clinical documentation and patient records

  • Supporting best practice diabetes management and preventative health initiatives within the clinic

About You 

To be successful in this role, you will have:

  • Relevant qualifications and accreditation as a Diabetes Educator

  • Current AHPRA registration or relevant professional registration

  • Excellent communication and interpersonal skills

  • A compassionate and patient-focused approach to care

  • The ability to work independently and collaboratively within a multidisciplinary team

  • Previous experience in general practice or community health (preferred but not essential)
