Jeremy graduated from UWA in 2012, and completed a diploma in Child and Adolescent health in 2015. Whilst being a Perth native, he also speaks Norwegian and German thanks to few years of high school in both countries.
Prior to starting work as a GP, he spent a number of years across department of paediatrics, emergency medicine, intensive care, cardiology, respiratory and gastroenterology in hospitals around Western Australia in addition to volunteer work in Malawi.
He is confident in all areas of general practice, in particular sports medicine, skin procedures, child and adolescent health, travel medicine and mental health.
Jeremy’s interests outside medicine include music, basketball, cycling, skiing and hiking.

Dr Mina Kamran Disfani is an experienced General Practitioner consulting at Floreat Medical, with over a decade of experience across hospital and community-based medicine. She completed her medical training in Western Australia and is a Fellow of the Royal Australian College of General Practitioners (FRACGP).
She has worked extensively within the Joondalup Health Campus since 2014, with registrar-level experience across general medicine, neurology, women’s health, paediatrics, mental health, emergency medicine, and chronic disease management. This broad clinical background allows her to provide comprehensive, evidence-based, and patient-centred care.
Dr Mina has a strong interest in women’s health, perimenopause and menopause, neurology and headache disorders, weight management, and chronic disease care. She places a high value on continuity of care and long-term therapeutic relationships, supporting patients through complex and evolving health needs.

My areas of expertise include women’s health, perimenopause and menopause management (as a member of the Australian Menopause Society), metabolic and chronic disease, diabetes, child health, sexual health, and minor procedures such as hormonal IUD/ contraceptive implants removals and insertions.
I also bring valuable additional experience from working within a GP specialist clinic, providing ADHD assessments and supporting patients with complex presentations.
As a long-term local resident and parent, I understand the diverse needs of families in the community. I enjoy caring for patients across all ages, with a particular interest in supporting women through perimenopause and menopause, and older adults with chronic conditions.

I am a local graduate of Medicine from UWA. My family moved to South West WA in 1976 from UK, where I did my initial schooling and being the daughter of a Surgeon, followed a career where I can enjoy caring for patients and their families.

Associate Professor Capolingua is passionate about General Practice being the most important interface and partnership with the patient for a healthy life, illness prevention, early detection and diagnosis, management and treatment. The role of the GP is to be there when you are sick, but more importantly, to be with you throughout life to keep you at your very best, physically and mentally. General practice is underpinned by careful assessment and focus on the patient as an individual in the context of family, community and society. Dr Capolingua has a focus on quality and the patient experience in the delivery of health care. She has interest across all medicine, all age groups and all needs and treats the patient as a whole person.
During her career, Dr Capolingua has combined her clinical work with a contribution to healthcare delivery in Australia. Her commitment has always been putting the patient first. This principle guides and delivers the best of care for everyone. She has had a strong public health focus with advocacy around enabling healthy lifestyles. She has developed broad experience across health care delivery having served as Federal President of the Australian Medical Association and also as State AMA (WA) President. Dr Capolingua has chaired the Federal AMA’s Ethics & Medical Legal Committee, the Taskforce on Indigenous Health and has been a member of 2 standing committees of the NH&MRC being the Human Genetics Advisory Committee and the Australian Health Ethics Committee.
Dr Capolingua believes in investing in younger colleagues and Floreat Medical is an accredited teaching practice, training WAGPET allocated GP registrars and teaching Medical Students from UNDA, UWA and Curtin University.
Dr Capolingua’s current positions are Practice Principal and owner of Floreat Medical. She is Chair of AMA Ltd, Chair of the Child and Adolescent Health Service WA, and Chair of the Western Australian Immunisation Strategy Committee. She is a Board member of St John of God Healthcare and Chair of it’s Patient Experience and Clinical Excellence Committee. Dr Capolingua is a member of the Board of Catholic Homes Inc, and Chairs the Clinical Governance Committee. She is also Medical Director of the AMA(WA) Youth Foundation.

I’m a passionate and thorough GP, experienced in paediatrics and family medicine.
After graduating from UWA in 2010, my medical career started in Fremantle Hospital, as Intern of the Year, where I worked a range of specialties. In 2014 I focused on paediatrics in Princess Margaret Hospital, where I completed my Diploma of Child Health. This broad range of medical training helps me provide thorough care for families at Floreat Medical. I have recently returned from working in the Mid-West to resume general practice medicine again in Floreat.
My active lifestyle and experience in the country reinforces the importance of an eye for detail and thorough approach to medicine, while recognising the important role of family and community. I hope I can also be a trusted member of your family too.
I work part time with appointments three days a week plus some Saturday mornings to fit around patients’ busy schedules.

I graduated from the University of Western Australia in 2010 with honours. I then worked in a wide range of hospital based disciplines including paediatrics, geriatrics, psychiatry, orthopaedics, coronary care, general medicine, general surgery and emergency medicine. Following this, I worked in travel medicine and as a clinical researcher for the Mc Cusker Alzheimer’s Foundation.
In 2016, I brought together all of my medical interests and capabilities to complete my Fellowship of the Royal Australian College of General Practitioners.
I can assist you and your family with all aspects of general practice across the lifespan- acute medical issues, chronic disease management, preventative medicine/general health checks, mental health care plans, GP care plans and team care arrangements.
Outside of the consulting room, I enjoy travelling, sport and being involved in local community projects and charity groups.

Hi, I’m Amanda. I’ve been in General Practice for more than 20 years and have been practicing medicine at Floreat Medical since 2011. After graduating from UWA, I set off interstate and overseas to gain experience and additional qualifications in the areas of Anaesthetics, Obstetrics, Gynaecology, Emergency and Rural Medicine. These skills have come in handy working in rural New Zealand, GP after hours at Subiaco, Murdoch emergency department and of course Floreat.
I work 4-5 days a week and am the mother of three amazing and very active boys. We all have the travel bug but our favourite holidays have been camping and 4 wheel driving with family and friends. Our love of the outdoors sees us enjoying and appreciating the wonders of nature as often as time allows.
I enjoy all that General Practice has to offer and consider it a privilege to be involved in the health care of people of all ages and all walks of life. I believe ongoing education and support of our current medical students to be very important and take the time to have students throughout the year.
I find it difficult to list my areas of special interest as I enjoy all aspects of general practice- paediatrics, woman’s health, men’s health, travel medicine, chronic disease, sports medicine, and weight management. Dr Lynn Graham completed her Medical training at the University of Otago, New Zealand in 2000 and worked in Emergency Medicine, Hospital General Medicine and General Practice prior to relocating to Perth in 2007.
In 2007 Dr Graham completed a fellowship training year at Fremantle Hospital in Hyperbaric medicine and is a member of SPUMS and performs both recreational and occupational dive medicals.
She has post graduate diplomas in both Sports Medicine and Hyperbaric Medicine and also has completed a Post Graduate Certificate in Skin Cancer Medicine.

Kate graduated from the UWA Medical School with Honours in 2008. During university she obtained the highest aggregate mark in Psychiatry units. Following graduation Kate completed two years residency at various tertiary institutions, including Sir Charles Gairdner Hospital, Joondalup Health Campus, Princess Margaret Hospital and King Edward Hospital, gaining experience in all aspects of medicine including critical care, paediatrics, obstetrics and gynaecology.
Kate has been working in General Practice since 2011 and obtained Fellowship of the Royal Australian College of General Practitioners in 2013. Following this she has worked as a RACGP examiner.
Kate enjoys providing care in all aspects of General Practice, with particular interest in paediatrics, antenatal care and minor surgical procedures.

Family Medicine and General Practice
