For item no. 23 you will be charged $72 and item n. 36 will be charged $115. Dr Carl Yan graduated from the University of Tasmania in 2014. Prior to working at Modern Medical Clinics, he has worked at Moora Health Centre in the Wheatbelt, Royal Perth Hospital, as well as Launceston General Hospital. Dr Yan is enrolled with the Western Australia General Practice Education and Training (WAGPET) programme where he is working towards completing his Fellowship. He enjoys all areas of General Practice, including family medicine. Dr Yan has a special interest in both Children and Women’s health. He holds a Diploma in Child Health, University of Sydney and Certificate of Women’s Health, RANZCOG.

Jemar is an Accredited Exercise Physiologist with postgraduate Master’s in Clinical Exercise Physiology training from the University of Western Australia and a Bachelor of Exercise, Sports, & Rehabilitation Science from Curtin University. He brings a broad clinical background, having worked across aged care, private practice, hospital, and university teaching.
With experience delivering exercise interventions under Medicare, NDIS, DVA, and WorkCover, Jemar is passionate about supporting individuals with chronic conditions and mobility limitations. He has a particular interest in chronic pain management, falls prevention, cardiac and metabolic rehabilitation, and behaviour change strategies grounded in evidence-based frameworks like ACT and CBT.
Jemar values patient-centred care and takes pride in tailoring exercise programs that are functional, empowering, and achievable. Outside the clinic, he also teaches geriatric rehabilitation to aspiring clinicians at the University of Western Australia.
